Fire safety record templates for facilities – fill in online
Three fill-in templates for companies, factories, warehouses, shops, restaurants and hotels in Vietnam. The documents are in Vietnamese because they are kept for Vietnamese fire police inspections; each section has guidance and a filled example for a steel warehouse.
During a fire safety inspection in Vietnam the facility must show its fire safety management records under Article 4 of Decree 105/2025/ND-CP. This set covers the three parts a facility drafts itself: the decision establishing the on-site fire team, the rules with logbooks, and the fire-fighting and rescue plan (Form PC06).

Căn cứ Luật Phòng cháy, chữa cháy và cứu nạn, cứu hộ số 55/2024/QH15;
Căn cứ Nghị định số 105/2025/NĐ-CP quy định chi tiết một số điều và biện pháp thi hành Luật Phòng cháy, chữa cháy và cứu nạn, cứu hộ, được sửa đổi, bổ sung bởi Nghị định số 347/2026/NĐ-CP;
Căn cứ Thông tư số 36/2025/TT-BCA quy định chi tiết Luật Phòng cháy, chữa cháy và cứu nạn, cứu hộ và Nghị định số 105/2025/NĐ-CP;
Căn cứ Điều lệ tổ chức và hoạt động của ……………………;
Xét yêu cầu bảo đảm an toàn phòng cháy, chữa cháy và cứu nạn, cứu hộ tại cơ sở.
QUYẾT ĐỊNH:
Điều 1. Thành lập Đội phòng cháy, chữa cháy và cứu nạn, cứu hộ cơ sở của …………………… gồm 1 người (có danh sách kèm theo). Đội trưởng: .
Điều 2. Đội có nhiệm vụ:
- Đề xuất ban hành nội quy, quy định về PCCC và CNCH; tổ chức tuyên truyền, phổ biến cho người lao động.
- Kiểm tra, đôn đốc việc chấp hành nội quy, điều kiện an toàn PCCC; phát hiện, đề xuất xử lý thiếu sót.
- Quản lý, bảo quản, kiểm tra phương tiện PCCC và CNCH; lập sổ theo dõi.
- Tham gia huấn luyện, bồi dưỡng nghiệp vụ; xây dựng và thực tập phương án chữa cháy và CNCH của cơ sở.
- Tổ chức chữa cháy, CNCH ban đầu khi có sự cố; hướng dẫn thoát nạn; phối hợp với lực lượng Cảnh sát PCCC và CNCH; bảo vệ hiện trường.
Điều 3. Đội trưởng chịu trách nhiệm trước Giám đốc về hoạt động của Đội.
Điều 4. Quyết định có hiệu lực kể từ ngày ký. Các bộ phận liên quan và các ông, bà có tên trong danh sách chịu trách nhiệm thi hành Quyết định này.

3Fire-fighting and rescue plan
Following Form PC06: site features, water sources, worst-case fire scenario, team assignments, other fire and rescue scenarios, revision and drill tables, plan drawings.
How to complete the records
- Read the general guide to see which documents your facility needs.
- Fill in Template 1 and sign the decision.
- Adapt the rules and post them at the gate, workshop and warehouse.
- Fill in the fire-fighting plan, draw the site plan, have the head of the facility sign it.
- Keep the logbooks up to date and file everything together on site.
